The following instructions only apply if your order matches these criteria:

  1. You selected Basic Shipping for your order.
  2. The package is delivered to an address within the United States.
  3. Delivery is done by United States Postal Service (USPS).

Please read this page carefully! Because the insurance company will reject any claim that is invalid or filed too late.

Important!

A damaged order claim must be filed with us within two business days, calculated from the moment of delivery.

Any claim that is filed too late or is filed with incomplete or invalid documentation will be rejected by our insurance company.

USPS step 1:

To prove that we packed and shipped your mirror according to the guidelines given to us by the insurance company, we need the following pictures and documents:

  1. Take a picture of the top-lit of the package, showing "Fragile - Breekbaar" warning tape.
  2. Take a close picture of the barcode/address label, located on the top-lit of the package. Barcode and address must be readable in the picture.
  3. If there is any visual damage on the outside of the package, take a detailed picture of it.
  4. Open the package by taking off the top-lit carefully and take pictures of the broken mirror while still in the package's bottom part.
  5. Download the USPS Form 2855 by clicking the PDF icon on this page and print it twice.
  6. Print the Mirror Engraving order confirmation PDF and Invoice PDF. Both send to you by email on the day you placed your order with us.
  7. On the USPS Form, complete sections 1 and 2 with your details.

USPS step 2:

  1. Now follow these USPS instructions below:
    1. If you receive an article that was sent from abroad that is damaged or has some contents missing, take it to any USPS Post Office™ in your area right away.
    2. Be sure to bring the item, the mailing container, wrapping, packaging, and any other contents.
    3. The Post Office will verify the damage and have you complete a claim form.
      (This is the Form 2855 that you already downloaded from our website).

  2. Ask the USPS employee to fill in all requested details under sections three (III - Completed by Post Office™ Verifying Receipt or Wrapper) and four (IV - Completed by Office of Address") on BOTH copies.
  3. Ensure that the "Case Number" at the upper right corner is also added to the form and the numbers 35c, 36, 37, and 38 are correctly filled in.
  4. One of the completed forms will stay with the USPS postal office. The other one is yours to take home for us.

USPS step 3:

When you completed all previous steps, scan the completed USPS Form 2855 (or take a picture of both pages).

Compose an email to [email protected] and attach:

  1. All pictures taken under step 1.
  2. The scan (or pictures) from the USPS Form 2855

As a subject, enter "Damaged order". In the body text, enter the following details:

  1. Your order/invoice number.
  2. Your name, full address (including zip code)
  3. Phone or cell number where you can be reached during office hours.
  4. The barcode number of the package (shown under the barcode address label of your package).
  5. The USPS case number on the USPS Form 2855
